Miami-Dade Public Housing and Community Development has made it easier for you to contact us with information about you. These forms can be downloaded, printed and faxed or mailed to the address on the top of each form.Applicant Leasing Center

If your address or family size has changed, we need to know! The head of household can complete the corresponding form and mail it to the Applicant Leasing Center. - Informal Review Request

If you have been denied housing and would like to exercise your right to an informal review, you must submit an Informal Review Request form.
Preliminary Interview

- completed by you listing all the addresses you have resided within the last five years - Landlord Reference Form
- completed by your landlord to include signature and contact information (address, and telephone number)
Direct Deposit
Direct deposit can help Section 8 landlords and accounts payable vendors save time by having funds immediately. It is a safe and convenient way to have your money deposited directly into your checking or savings account. To enroll in direct deposit, download and fill out the form and mail it to the address on the form.

Reasonable Accommodation Request

To be used by applicants or program participants to request a reasonable accommodation. It also provides examples of reasonable accommodations (also available in Spanish

To be used by the health care provider to verify the person's functional limitations and explain how the accommodation will assist the person. Not used when a live-in aide is requested. - Live-in Aide Verification

To be used by health care provider to verify if the person requires a live-in aide. - Authorization to Release Information

To be signed by the person with disabilities authorizing the health provider to release disability verification information (also available in Spanish
and Creole
).
